The Oaks PACE clinic








Transportation is provided by our own friendly staff members at The Oaks PACE

The Care and Services Included in this Program:

• Nursing; physical and occupational therapies; meals; nutritional counseling; social work and personal care
• Medical care provided by a PACE physician familiar with the history, needs and preferences of each participant
• Home Health
• Social Services
• Recreational therapy
• Medical specialties, such as audiology, dentistry, optometry, podiatry, speech therapy and others
• Respite care
• Hospital and nursing home care when necessary
• Transportation
• End of life
• Pharmacy services

When enrolled in a PACE program, all services including primary care and specialist physician services must be furnished by or authorized by the PACE provider.  If services are not authorized by the PACE provider, then the participant may be fully liable for the costs of unauthorized or out of PACE program agreement services.
